Referrals
The online referral button opens a Google Form managed through our business Google Workspace account. Information you enter is used to review your request and contact you about services. Authorized practice staff review responses. Submitting the form does not establish care or confirm an appointment.
Please provide only the information requested. Do not include Social Security numbers, insurance identification numbers, or detailed medical records. The downloadable PDF is not automatically submitted; call us to arrange a secure way to return it.
Community pet photos
Pet-photo submissions store the photo, pet’s name, caption, and submission date for review. Photos remain unpublished until the practice owner approves them. By checking the permission box, you allow us to display the approved photo with its pet name and caption publicly.
Please submit pet photos only. Do not include client information, identifying details about other people, or photos of people without their permission. Call us if you want your submitted photo removed.
Website management and referral sources
Unpublished blog drafts and photos awaiting review are restricted to the practice owner’s account. If you use the optional referral-source selector, the website records the source category and date; it does not request clinical information.
Other services and links
This website uses hosting, storage, and account sign-in services to operate. The referral form and linked external websites use their own services and privacy settings. This page describes website submissions; it is separate from the clinical privacy information provided when you begin care.
